- Incorrect Mullion Installation: The mullions were installed backward, leading to extra holes in the mullions, compromising both aesthetics and functionality.
- Improper Door Closer Installation: A closer had been installed on a Special-Lite door, resulting in four holes drilled into the door and frame.
- Broken Screw and Exposed Hardware: One screw had broken off inside the door after hitting a truss rod, leaving four screws exposed and affecting the integrity of the door.
- Missing Exit Device Trim: Despite being essential for the installation, device trim had not been provided or installed.
